All Lincoln University of Missouri students are required to submit their health records to be fully admitted as a student. You can conveniently upload your records through the Lincoln Univeristy Medical History Form below:

Health Record Requirements
All incoming students for Spring 2025 are required to submit their Medical Requirements through the LU Medicat Student Health Center Portal by January 3, 2025.
All incoming students for Fall 2025 are required to submit their Medical Requirements through the LU Medicat Student Health Center Portal by August 1, 2025.
Immunization Requirements
Students must provide the following health records to Thompkins Health Center to be fully enrolled at Lincoln University of Missouri.
- Lincoln University Medical Health History Form
- Immunization records must include proof of two (2) MMR vaccinations (Measles, Mumps, Rubella).
- Meningococcal Vaccine
- Students living on campus in university owned residential housing must provide documentation of receiving one of the following meningococcal vaccines within the last 5 years: Men ACWY or MCV4 (Menactra, Menveo) or MPSV4 (Menomune).
- If your first dose was before the age of 16, then a booster is required to live in on campus housing.
- Please note that a Meningitis B (Bexsero) is not the same as the above, and while recommended, is not required.
- TB Skin Test or Chest X-Ray: Students must have a TB skin test dated within the last 12 months or proof of chest x-ray demonstrating negative TB results. This is an annual requirement for students enrolled at Lincoln University.
- Please complete the TB screening form if the following does NOT apply to you.
- I am from, were born in, or have visited for more than 2 months Asia, Africa, Central or South America, Caribbean, Eastern Europe, or Oceanic/Pacific Islands.
- I have been diagnosed with a chronic medical condition that may impair my immune status.
- I am or have been a health care worker.
- I have been a volunteer or an employee of a nursing home, prison, homeless shelter, AIDs facility or other residential institution.
- I have close contact with someone who has or previously had active TB.
- Students can upload electronic copies of their immunization records and TB Skin Test to Medicat, which is on their student portal.
- Please complete the TB screening form if the following does NOT apply to you.
Students may provide copies to Thompkins Health Center via fax at (573) 681-5877 or hand deliver copies to 822 Lee Drive while they are on campus.
If you have difficulty attaining your medical records, please come to Thompkins Health Center, where we will be happy to help you locate and request your health records.
